Section 2: Print/Read/Keep the DOCUMENTS below:

a. Chaperone Training 

b. Student Discipline Matrix  

c. Complete TB Test and Fingerprints* (for Non-District Employees)

 

Section 3: Print/Complete/Return the DOCUMENTS below to the CHS, Student Services Office:

a. Chaperone Agreement Form

b. TB Results (once received from your Dr.)

c. Fingerprints (District keeps this information and gives results to CHS Office)

 

Section 4: *ONLY IF DRIVING* If you are driving on the trip Read/Print/Complete/Return the documents below to the CHS, Student Services Office:

a. Driver Information Form

b. Driver Information Sheet

c. Non-District Driver Private Vehicle Form

d. District Employee Private Vehicle
